All fields marked with a red asterisk * must be filled in and will only be used by FCC staff to validate your request. Only the Domain Name will be seen by the public.

Here is a description of the fields:

  • Request Type: This can be a new domain name, an update to an existing domain name, or a request to delete a domain name. The most important field on this screen is the Domain Name.
    If you are changing or deleting an existing domain name, be sure to enter the domain name exactly as it is in the FCC database. If there are typos in the domain name, the request type should be 'delete' and the domain name, including typos, should be entered as it exists in the database. If the domain name has been discontinued, the request type should be 'delete'.  
  • First Name: Enter up to 40 characters for this field.  
  • Last Name: Enter up to 40 characters for this field.  
  • Organization(CMRS provider): This is the expanded organization name for the CMRS provider. For example, instead of entering FCC, enter Federal Communications Commission. Enter up to 100 characters for this field.  
  • Phone Number: Enter up to 20 characters for this field. Please enter the phone number in (aaa) 123-4567 ext: 1234 format. If there is an extension, please enter it.  
  • E-mail Address: Enter up to 50 characters for this field.  
  • Address 1: Enter up to 50 characters for this field.  
  • Address 2: This field is optional. Enter up to 50 characters for this field.  
  • City: Enter up to 50 characters for this field.  
  • State: Select the state from the list of available states.  
  • Zip: Enter up to 10 characters for this field.  
  • Domain Name: Enter up to 75 characters for this field.  
  • Approve: Please read the legal notice and check this circle. This request cannot be submitted until this circle has been checked.
